11 international fellows at the Vanderbilt University Medical Center Center for Health Economic Modeling used our CEA Registry to conduct cost-effectiveness analyses (#CEA) relevant to public health their home countries. The fellows, all government health economists in low- and middle-income countries (#LMICs), spent three weeks learning about decision science and health economics and conducted a CEA as a capstone project. From the article: “One of my proudest achievements during the fellowship was being able to design a cost-effectiveness model focused on reducing adolescent alcohol use in Uganda — a pressing public health issue back home,” said Roderick Atuhaire, principal policy analyst in the Uganda Ministry of Health. “I returned to Uganda not just more skilled but more strategic, driven and determined. The program instilled a strong work ethic, a sharper analytical lens and a profound belief in the power of data to shape better futures.” CEVR’s Peter Neumann gave a seminar on cost-effectiveness analysis and Xiaoyan Wang traveled to Nashville to demonstrate how the CEA Registry can be used to quickly find, export, and extract data from relevant studies. Disease Control Priorities, Fourth Edition (Volume 2): Investing in Pandemic Prevention, Preparedness, and Response Editor(s) Sharma, Siddhanth Bertozzi, Stefano M. Fan, Victoria Y. Jamison, Dean T. Norheim, Ole F. Oshitani, Hitoshi Pate, Muhammad Ali Abstract: The fourth edition of Disease Control Priorities (DCP) builds on the three previous editions published from 1993 through 2018. DCP4 uses a collaborative, country-led approach to summarize, produce, and help translate economic evidence to inform better priority setting and capacity strengthening for universal health coverage, public health functions, pandemic preparedness and response, and intersectoral and international action for health. New modeling for DCP4 suggests more than a 20 percent probability in the next decade of another pandemic with mortality comparable to COVID-19. Volume 2, "Investing in Pandemic Prevention, Preparedness, and Response", provides practical guidance for countries to prevent, prepare for, and respond to pandemics. Drawing on COVID-19 lessons, it identifies key priorities to build resilient systems that protect lives and economies through evidence-based national and regional actions. Early chapters focus on prevention—reducing zoonotic risks, strengthening biosafety measures, improving early detection systems, and rapidly characterizing and containing outbreaks. Other chapters discuss targeted public health measures and broader public health and social measures to curb transmission. Core health system investments receive particular attention, especially oxygen systems, emergency care, and vaccine delivery infrastructure that serve both routine and pandemic needs. Later chapters address medical countermeasures, proposing prearranged financing and decentralized manufacturing to accelerate deployment and reduce disparities. Finally, the volume presents a comprehensive framework for pandemic financing that spans the entire pandemic cycle, emphasizing the need for timely, adequate, and effective financial resources. The framework is designed to support policy makers in countries at all income levels and provides a guide to appropriate financing tools for each stage of a pandemic. The President's budget directs NIH to reserve half of its competing research program grant award budget for multi-year funding (MYF). Without additional policy changes, transitioning to MYF will temporarily reduce the number of new research program grant awards (e.g., RO1, P30, UM1) that NIH can issue. The biomedical research community needs to advocate for a transition to MYF that maintains robust NIH funding for new, cutting-edge science. I appreciate the NIH's awareness of this issue and willingness to consider innovative solutions. For those who want additional detail on MYF of awards... Multi-year funding is an approach used by some federal funding agencies (e.g., the National Science Foundation). It does not change the availability of funds for principal investigators; it is simply an accounting strategy. With MYF, all years of the research project grant award are allocated to the awarding year's federal budget. For example, under MYF a four-year RO1 ($250K/year) would require allocation of $1 million in NIH dollars the year it was awarded. Under the current budgetary process, only the RO1's first year budget ($250K) would be allocated to the year it was awarded, allowing the NIH to fund three additional new RO1s that year for the same $1 million. The justification given by the administration for moving to a MYF accounting process is that MYF will "increase NIH budget flexibility by no longer encumbering large portions of each year's appropriation for the continuation of research projects that were initiated in previous years". On Capitol Hill today with colleagues from the American Society for Biochemistry and Molecular Biology (ASBMB) advocating for strong federal investment in basic science. Lots of meetings, lots of walking, and lots of enthusiasm! As someone who teaches biochemistry and mentors undergraduates in research, I see every day how funding from agencies like the NIH and NSF creates opportunities for students to do real science, and imagine themselves as the next generation of scientists. Stable, sustained research funding matters. For discovery, for innovation, for the students coming up behind us, and for our communities. #ASBMBAdvocates #ScienceServesUsAll

As 2025 federal research funding results come into focus, we’re starting to see institution-level “winners and losers” lists (e.g., GEN’s Top 50). These are useful, but they can also be misleading because they conflate 2025 dollars with out-year commitments.
