The shift is happening across three fronts—policy, standards, and enforcement: ✔️ HHS is moving to accelerate AI in clinical care (regulation, reimbursement, R&D). ✔️ The Joint Commission + CHAI are setting the standard for “responsible AI”—governance, validation, monitoring, transparency. ✔️ California is raising the bar on disclosure and limiting claims that AI can act like a licensed clinician. Translation: Adoption is no longer the bottleneck. Accountability is. What most teams are missing: You don’t need another AI strategy deck. You need to prove your systems are safe, explainable, and compliant—before they scale. Concrete example A health system evaluating an AI triage tool isn’t just asking, “Does it work?” They’re asking—often under a compliance review or procurement deadline: ✔️ How was it validated—and on which populations? ✔️ What happens when it fails? ✔️ Who owns oversight? ✔️ Can we defend this to regulators and auditors? If you can’t answer those clearly, the deal stalls—no matter how strong the model is. What stood out most: The life sciences industry is at an inflection point on compliance monitoring. The organizations that are making real progress are the ones treating monitoring as a continuous, risk-prioritized discipline — not a periodic exercise. They're asking the right questions about which activities carry the most risk, making those risks measurable, and then driving actual behavior change from the findings. Mobile device compliance was a recurring theme throughout the conference. The debate between restricting communications and enabling them with proper archiving and monitoring technology is one most organizations are still working through — and the pressure to get it right is mounting. The DOJ's ECCP guidance, and recent settlements make that clear. The next 12–18 months will be telling. I expect to see more life sciences firms move decisively toward technology-enabled compliance — particularly around mobile and social media — as the regulatory environment continues to raise the bar. That sounds harsh, but it is what is happening in a lot of enterprises. One team buys an AI monitoring tool. Another adds a model card process. A third creates a risk register. Legal keeps its own tracker. Security adds another dashboard. Everyone is trying to do the right thing. But the result is often the same: fragmented ownership stale evidence conflicting definitions manual reconciliation no single view leadership can trust That is not governance maturity. That is governance sprawl. The uncomfortable truth is that AI governance will break faster than traditional compliance if it gets patched together with more point tools. Because AI moves too fast, changes too often, and creates too many cross-functional dependencies for static tracking and disconnected systems. At some point, the spreadsheet stops being a shortcut and starts becoming the risk. If this sounds familiar, you are not alone. Andrew Yinger reacted on thisAndrew Yinger reacted on thisThe next control failure won’t come from a person ignoring policy. It’ll come from software taking initiative nobody explicitly approved. That is the real issue with agentic AI. Not that it writes text. Not that it summarizes meetings. But that it can observe, decide, trigger actions, call tools, move data, and keep going unless somebody designed limits into the workflow. The EU AI Act defines AI systems as machine-based systems designed to operate with varying levels of autonomy. LockThreat makes a similar point more bluntly: AI systems and agentic agents introduce new risk, while evidence is now expected in real time, not just at audit time. That should change how leaders think about control. Most organizations still govern AI as if it were just a content tool. But agents create a different class of problem: delegated decision-making, unclear approval boundaries, hidden data movement, and weak accountability when something goes wrong. If an agent can take action inside enterprise systems, then “who approved the prompt?” is not a serious governance question. The serious questions are: - Who approved the objective? - Who defined the boundaries? - Who reviews the evidence trail? - Who is accountable for downstream harm? If you cannot answer those four questions, you do not have agentic AI governance. You have optimism with permissions attached. Autonomy without oversight is not transformation. I am a meal provider for the women's shelter on a monthly basis, preparing fresh, wholesome meals for women at one of the worst times of their lives. It is a blessing to serve in this way and praying over the meals with these women before their dinner is the highlight of most of my days cooking for this very important local organization.
