IT is not a Cost Center. Or it should not be. If is a cost center, all you have is desktop support but business leaders want more. Now the inverse. IT will not be come a true Profit Center for most businesses. Offering an IT product or service would be outside the businesses core offering. The objective is to make IT an Accelerator. The IT function should be boosting operations, sales and customer support to increase efficiency, customer support and profits. I am a fractional CIO, who partners with CEOs and CFOs, who are responsible for IT, and don't feel lT is keeping up with business demand. Maybe IT even feels like a black hole. I can create a 90 day plan to get IT meeting demand and on the path to being a business Accelerator.

I think it's time we start talking about the elephant in the room in healthcare. Over 100 hospital systems led by CHIME are fighting to kill the proposed HIPAA Security Rule overhaul. The headline number they keep throwing around? $9 billion in first-year compliance costs. I have questions about that number. We work with small and mid-sized healthcare organizations every day — practices, clinics, specialty groups — and we're implementing these exact controls right now. MFA. Encryption. Asset inventories. Pen testing. Risk assessments. Incident response. We're doing it on Kool-Aid budgets, not champagne ones, because that's who we built our services for. Organizations that need enterprise-grade security but don't have enterprise money. So when I hear $9 billion, I have to wonder — who's pricing this out? Because if a 50-person practice can get compliant with our tools and fractional CISO support for a fraction of what the big consulting firms charge, maybe the cost argument is more about protecting the status quo than protecting patients. And here's the thing — this is happening whether the NPRM survives or not. A bipartisan group of Senators — Cassidy, Warner, Hassan, Cornyn — already reintroduced the Health Care Cybersecurity and Resiliency Act with the same core mandates: MFA, encryption, annual pen testing, cybersecurity audits. Paired with federal grants to fund it. New York didn't wait at all — they enacted mandatory cybersecurity regulations for every general hospital in the state. CISOs, annual risk assessments, penetration testing, 72-hour incident reporting. Full compliance hit October 2025 with $500 million in state funding behind it. Other states are already drafting their own. The direction is set. The only variable is the timeline. So why are we fighting this? This isn't bleeding-edge stuff. This is basic security hygiene that every organization handling patient data should already have in place. 275 million healthcare records were breached last year. 238 ransomware attacks on healthcare. The Change Healthcare breach alone hit 190 million people. At some point we have to stop treating cybersecurity like a regulatory burden and start treating it like what it is — patient safety. If your organization needs help getting ahead of this without blowing your budget — that's literally what we do. We make enterprise security accessible for organizations that were told they couldn't afford it. Reach out. As promised... a bit deeper dive into the 4 levels (Operations, Administration, Engineering, Architecture): Operations: Operations agents/techs or whatever you call them are often the front line to the customer or end user. They work in the help desk, they go on-site, they perform deployments (in some orgs), they do a lot of the front-end work. 80% of all support requests should be solved by the operations team. These folks use the instructions provided by the levels above them, to manage, fix, change and execute process. When you call a help desk, you should be speaking to someone in operations, who can actually 'do' something about your issue. Operations is not: Choosing the equipment to use, defining/engineering the stack, writing policy and procedure, or building business cases (though they can assist). Day-to-day, operations is tough. They are the ones getting yelled at by the client/customer, they are the ones owning bringing things to a resolution, they are the ones in front of others doing the 'work'. Operations teams turn to Administration teams when they cannot solve a problem. They may also turn to vendors, or higher-level teams if Administration teams are unclear. These folks are the ones who produce the data to measure KPIs. (They don't define the KPIs, they measure them). They escalate when their skill is exceeded, and they follow up throughout a service issue lifespan to keep things moving... They are the ones who wake you up at night to tell you something is broken and they can't fix it... the Network Operations Center teams, the Security Operations Center teams, the Help Desk, the front lines... no matter what you call them... Often these positions start as entry-level and develop skills, learn systems and then progress to Administration level systems. Next up... how to differentiate with Administration teams...

Healthcare CIO’s Brief: Healthcare IT Security—Where We Stand Now 1. The Current State: Mounting Threats and Supply Chain Risk In the first half of 2025 alone, breaches impacted more than 29 million patient records via some of the largest data compromises reported to HHS’s OCR portal. Much of this stems from hacking and IT incidents; back in 2010, only 4% of breaches were due to hacking—today, that figure has surged to 81%. Ransomware remains a constant threat: 77% of healthcare organizations were targeted in the past year, with 53% of those attacks succeeding. And the ways attackers strike keep expanding—supply‑chain attacks compromised data of over 78 million individuals in the first half of 2025. 2. The Price Tag: Still the Most Costly Sector #Healthcare retains the unenviable title of the costliest industry for data breaches. IBM’s 2025 report places the average cost per breach at $7.42 million, down from $9.77 million last year, but still the highest across all sectors. A breakdown of costs reveals detection and escalation ($1.47M), lost business ($1.38M), and post-breach response ($1.2M) as the main expense drivers. Notably, healthcare breaches take the longest to identify and contain—279 days on average, nearly five weeks more than the global average. #cio #ciso

This statement on information blocking could disrupt EHR vendors’ plans to corner the healthcare data exchange market. Here’s some quick background: EHR vendors like Epic, eCW, and Cerner set up (or are setting up) Qualified Health Information Networks (QHINs) that connect with other QHINs to form a national data exchange network. Healthcare provider organizations must decide whether to participate in the national network, and if they do, whether to join the QHIN that their EHR vendor offers or an independent one. It would have been tempting for the EHR+QHIN vendors to strongarm their provider clients into proprietary QHINs. The EHR vendors could have said things like: 🫸 “Choose our QHIN or you will be on your own to bring data from the national network into the EHR.” 💰 “You will have to pay us so much to ingest data from a competitor QHIN that you might as well join ours.” 🏆 “Joining our QHIN may be costly, but it would help you qualify for our Super Amazing Client Program and avoid costly EHR penalties. And we’ll waive our QHIN costs for a year or two.” ⏳“You could submit a request to bring data from a competing QHIN into the EHR, but it’s not really a priority for us and could take a while. Just join our QHIN.” The section of the FAQ document pictured below says that attempts to limit their customers’ ability to select a QHIN would be considered information blocking. EHR vendors that engage in such behavior could face monetary penalties and legal consequences. I agree with this guidance from ASPT/ONC. Providers and other organizations should be able to select the QHIN that offers the best services at the lowest price - not the QHIN that has the most leverage. EHR vendors that offer QHINs already have systems to connect with other QHINs to bring data into the EHR from the national network. They should allow their provider partners to bring life-saving clinical data into the EHR without forcing them into their QHINs. I’m looking forward to seeing whether the four-sentence FAQ response from ASTP/ONC will change the direction of the market on this. Perhaps you are reading this differently. What do you think?
