I took ~a month mostly unplugged with my family before writing this, and wanted to take a moment to say a proper thank you. The people made Nerdy a special place. I had the chance to work on big, exciting projects and spend time in just about every corner of the business. I learned a ton, laughed a lot, and was lucky to work alongside folks who believed in me and pushed me to grow. I’ll be cheering on the Nerdy team and all that’s still ahead. You’ve built something meaningful, and it’s been an honor to be part of it, Chuck. For now, I’m making up for lost time - spending more days with my kids, walking the dog, and enjoying lunch dates with the wife. If we worked together, please stay in touch!

It just wasn't delivering the accuracy or the spirit of what I wanted. Then my daughter called me into her room. She wanted to show me a drawing she made in honor of what would have been her brother's 12 birthday. She based it on one of the few photos we have of him. I was moved beyond words and it's priceless to me. Some things, like intent and love will give human creations intrinsic value.

Just wrapped up Impact 2026 in Denver, and it was an incredible few days with our network. We brought together owners and leaders from across North America to focus on one thing: driving impact—together. We discussed the direction of our industry, the scale of demand in front of us, and the responsibility we have to lead the future of aging at home. However, what stood out most wasn’t the strategy. It was the people. The Care Pros who show up every day with compassion and purpose. The owners and leaders building great businesses while making a real difference in their communities. The teams across the network continuing to raise the bar. Over the past few years, we’ve been building something meaningful—investing in data, strengthening our operations, and creating greater alignment across the network. Health systems are rarely tested by a single disruption. More often, pressure accumulates quietly — rising supply expense, workforce constraints, reimbursement compression, and growing expectations to do more with less. Over time, these forces reveal an important truth: Organizational resilience is less about reacting to events and more about how the system was designed to operate in the first place. Leadership teams increasingly face a defining question — not whether disruption will occur, but whether their operating model is durable enough to absorb sustained pressure. Supply chain sits closer to that answer than many realize, serving as both an operational backbone and an early indicator of organizational strain. Durable organizations are built intentionally. Less is More in MSSP Now that the 2024 MSSP results have been out for a few weeks, one trend stands out clearly - smaller ACOs continue to outperform their larger counterparts in per-beneficiary savings. When reviewing the 2024 data, ACOs with fewer than 35,000 assigned beneficiaries consistently generated higher per-beneficiary savings compared to larger organizations. While scale has its advantages; broader infrastructure, diversified risk pools, and negotiation leverage, it can often comes at the cost of clinical precision and local accountability. Smaller ACOs, on the other hand, tend to maintain tighter physician alignment, faster feedback loops, and deeper visibility into care coordination opportunities. As MSSP continues to evolve, it’s a reminder that bigger isn’t always better. Success in value-based care often comes from focus, engagement, and agility - not just scale.

Sometimes the biggest barrier to optimization isn’t data or resources... it’s fear. Fear that improving a process might look like admitting we “weren’t doing it right” before. Fear that asking questions implies someone dropped the ball. But the reality is this: Most of us inherit processes. They work. They got patients through tough moments. And that matters. Working, however, isn’t the same as optimized. Healthcare evolves. Reimbursement rules change. Documentation standards shift. What was once sufficient may no longer reflect the true complexity, risk, or cost of care being delivered today. That’s especially true in ECMO. Financial optimization around ECMO billing and coding isn’t about pointing fingers or rewriting history. It’s about ensuring today’s care is accurately captured, supported, and reimbursed so programs can remain sustainable, staff can be supported, and patients can continue to have access to this life-saving therapy. Moving forward isn’t an admission of failure. It’s a sign of maturity, stewardship, and leadership. Optimization says: We care enough to do better...not because we were wrong, but because we know more now. Not all data is created equal. Most teams track way too much. They drown in data and miss what really matters. In a large company like Amazon, the more metrics the better, because those operations are incredibly complex. In a smaller startup you need to keep things much more focused. Otherwise you'll lose the chance to make real progress and be stuck fighting all the daily fires. Here’s how I focus my teams: Define the Big 3. What are the three metrics that tell us how well we’re executing on strategy? Establish leading indicators. Don’t wait for monthly reports to find out something’s off. Track real-time metrics that predict outcomes. Review, refine, repeat. Metrics are only powerful if they’re accurate and relevant. Every quarter, we assess what’s still valuable and what’s just noise. Execution is a lot simpler when everyone’s looking at the same scorecard. NSA PSA: Qualifying Payment Amounts aren’t as “neutral” as you might think. QPAs are payors’ supposedly unbiased benchmarks for calculating accurate rates… except, a new study shows, they are not. In fact, payors are typically wildly understating them. Here’s one stunning example from the study. An insurer set its QPA at $0.01 (yes, 1 cent), but the IDR process found that the real number should actually be $1,196. In other words, a difference of 120,000%. Not precisely a clerical error!
