Giulia Block is our voice in the room - fighting to make high-quality care accessible, affordable, and protected by lasting reform. Too often, healthcare decisions are made based on numbers and policy metrics, without considering the outcomes those decisions create. And it’s tough, especially when models of care that are proven to work are still tied to policies created in response to a temporary emergency. That’s where Giulia does the heavy lifting. As our General Counsel, Giulia wears many hats. But one of the most impactful is the one she wears when she’s in Washington, D.C., advocating on behalf of every provider and patient who’s benefited from virtual care. She’s been deeply involved in policy work through the American Telemedicine Association (ATA), and just recently traveled to Capitol Hill to meet with members of Congress on both sides of the aisle. A big part of her focus right now is advocating for reimbursement models that don’t rely on public health emergency extensions. These temporary waivers served their purpose during the COVID pandemic, but virtual care has proven its value far beyond that context. Now it’s time for the policies to evolve because the care model is here to stay. Giulia’s work helps lay the foundation that allows clinicians to focus on what matters most - caring for patients, not navigating red tape. It’s easy to overlook this kind of work because it’s behind the scenes. And it doesn’t always come with headlines or high-fives. But the stakes are enormous. So thank you Giulia, for being the voice of Access TeleCare as we work to shape digital health and virtual care policy. We’re grateful to have you leading that charge.

Karina Akib liked thisKarina Akib liked thisAI in healthcare has become a hot topic. But no technology, not even AI, will have much of an impact if it never gets adopted by caregivers. Adoption, in any industry, is always the real challenge. As a CMO, I spend a lot of time thinking about how things actually show up in clinical workflows. The question isn't whether AI and digital tools can help. Its what actually changes provider behavior so they’re used consistently in real care. This past week I had the opportunity to participate in a session on this topic with the U.S. Department of Health and Human Services (HHS), the Office of the National Coordinator for Health IT, and the American Telemedicine Association (ATA) in Washington, D.C. There was a lot of thoughtful conversation around making AI feel genuinely assistive at the point of care, not like another layer. And just as important, how policy and reimbursement can support a shift from episodic visits to something more continuous and connected. I also appreciated the focus on access - done right, these tools can extend care in meaningful ways without widening gaps. Overall, a very practical, grounded discussion. Encouraging to see the focus not just on innovation, but on what it actually takes to make it work.

Transforming healthcare with AI requires industry. Industry requires capital investment. Capital investment requires return. Return requires revenue. Revenue requires reimbursement. Medicare (and transitively private insurance) reimbursement policy for clinical AI will define the incentives for the technologies we build and deploy over the next decade. Our existing fee for service codes are what the military would call cost-plus contracting. We pay for every nail, screw, and hour of labor and don't care if that was a good and efficient way to build the tank or not. Our CMS Innovation Center director, Abe Sutton, believes cost-plus is the wrong path for the future of American healthcare. ACCESS and VBC models pay for outcomes. When we have AI that can manage heart failure can we link payment to making people healthier? Doing so would unleash the might of American techno-capitalism on exactly the thing we care about in society, but there are devils in the details.

It had to be said! 🎯 Epic's stranglehold on healthcare IT has stifled innovation for too long and the writing is on the wall. The consolidation myth needs to die already. We were promised efficiency and lower costs, but what we got was bloated bureaucracies that prioritize profits over patients. The data doesn't lie: smaller, agile providers consistently outperform these healthcare monopolies on both outcomes and patient satisfaction. Time for a major reset in how we think about healthcare delivery.

More data in health care does not always mean better outcomes. Most consumer data only appeals to the worried well, without considering the downstream impact on the health system. Real change requires real ROI - justified reimbursement, workflow changes, and overall cost reduction. Data is only valuable when it changes behavior, not just for patients, but for payers and providers too. The founders who win will connect consumer insight to system economics, where the real leverage lives.

Longevity isn’t about living longer at any cost, it’s about living better for longer. Yet, the gap between healthspan and lifespan in the US is the highest in the world and has grown to over 12 years. This gap is 29% higher than the global average and means that, in general, we are living over a decade of life in a diseased state. I spend a lot of time thinking about how this shift from reactive to proactive care is evolving and what emerging strategies from the elite performance world can be applied to accelerate this transition. The focus becomes less about how long can we live, and more on how long can we maintain strength, cognition, metabolic health, and independence? I’m really looking forward to this conversation with three outstanding people operating in this space.
