If you’re building a health tech startup, here’s a useful perspective from the accelerator side. At ViVE, I spoke with Keith Camhi, Managing Director of the Techstars Healthcare Accelerator powered by Permanente Medicine Mid-Atlantic States. The program invests in early-stage startups and helps founders run pilots with physicians in the Kaiser Permanente ecosystem. Some interesting takeaways: • Techstars still prioritizes “Team, team, team” above everything else; • Founders in the program are interviewed by physicians before acceptance; • The goal isn’t just mentorship — it’s getting real pilots with providers; • AI startups dominate applications, but the focus remains on real clinical problems. Dashboards don’t create accountability. They create meetings. Charts get shared. Everyone nods. Action items get “noted”. Same problems. Next quarter. Accountability doesn’t come from visibility. It comes from systems — where ownership is clear and decisions can’t be ignored. I wrote a short note on why dashboards feel productive but rarely change behaviour — and what actually does. San Francisco-based Onos Health has raised $6.3 million in an oversubscribed venture round to transform how health plans manage behavioral health. Onos Health is a healthcare AI company tackling the estimated $60 billion lost each year to care misalignment and administrative waste. Its platform uses proprietary AI models to analyze payer data in real time, helping health plans identify optimal care pathways, eliminate outdated processes like prior authorizations, and direct more dollars toward high-quality care. The round was co-led by Haystack and Pathlight Ventures, with participation from pre-seed investors Bertelsmann Healthcare Investments and Nebular. The funding will be used to support growing demand from the nation's largest health plans and to expand its clinical and data science teams. We’ve been talking with a lot of enterprises about adopting AI. Some are all in. Others are navigating the risks, particularly around accuracy and reliability. In a recent conversation with healthcare leaders, Faisal Khan, CCSK, ISO Lead Auditor made a point that stuck with me: AI risk isn’t just about prompts, settings, or bad actors. It’s also about data integrity. If the underlying data is incomplete, conflicting, or just wrong, the output will be too. It was a good reminder that our “older” principles around data integrity and governance matter even more in an AI world.

An example of how California pursues fraud cases in health care ⤵️ 🏥 The Case: California v. Sweetwater Care On June 24, 2025, California Attorney General Rob Bonta filed a lawsuit against Sweetwater Care, a San Diego-based operator of 19 skilled nursing facilities. The allegations: ⚠️ 14,126 instances of staffing below California's minimum legal requirements (2020-2024) 💰 $31 million extracted as "profit" or "management fees" instead of funding legally required staffing 💵 $299 million received in Medi-Cal and Medicare payments during the examination period The patient harm documented in the complaint: ➡️Fractured bones that went days without assessment or medical care ➡️Patients with head trauma leaving facilities unbeknownst to staff ➡️Pressure injuries so severe a patient's hip bone was visible ➡️Medical emergencies not timely assessed or responded to ➡️Patients left overnight in soiled diapers because staff were too few 🎤 What the Attorney General Said "Sweetwater and its skilled nursing facilities violated the law and betrayed the trust of communities by failing to safeguard the health and safety of its residents. This is simply unacceptable." "We are holding Sweetwater accountable for breaking the law by understaffing its facilities and leaving residents vulnerable to serious neglect and injuries. No one is above the law, and our vulnerable patients deserve nothing less than dignity, safety, and high-quality care." 💡 The Funding Question Here's what makes this case instructive for the broader enforcement landscape: The investigation was conducted by California's Division of Medi-Cal Fraud and Elder Abuse (DMFEA). From the press release: "The Division of Medi-Cal Fraud and Elder Abuse receives 75 percent of its funding from the U.S. Department of Health and Human Services under a grant award totaling $69,244,976 for Federal fiscal year (FY) 2025. The remaining 25 percent is funded by the State of California." 🔄 Federal dollars funding state enforcement of facilities receiving federal program payments. This is one of the reasons people are so riled up from an enforcement perspective. Funding is federal. Accountability is state. This happens all across our system (not just in healthcare), and the impacts are deeply personal. We just released a new episode of Stedi's 'Understanding Healthcare with Zack Kanter' podcast featuring the inimitable Brendan Keeler. Brendan is the Interoperability Practice Lead at HTD Health and the author of the excellent Health API Guy Substack (if you listened to the Acquired podcast about Epic, you probably heard his blog referenced a dozen times). It was a blast diving into a broad range of topics related to healthcare interoperability – why healthcare transaction exchange is at such a tremendous advantage to other industries due to standardization, FHIR and information blocking, and how the shifting landscape of AI calls into question the role of standards going forward. One takeaway that resonates strongly with me is that AI is shifting chronic disease care from reactive visits to proactive, personalized, and continuous support, unlocking tremendous near-term, system-level opportunities for outcome improvement and cost savings.
