Anshuman Das liked thisAnshuman Das liked thisSharing reflections from my talk at ASOOCON 2026 on what the evidence actually says about secondary cytoreductive surgery. Secondary Cytoreduction in Recurrent Ovarian Cancer: Who Actually Benefits? Three randomised controlled trials, GOG 213 in 2019, found no OS benefit, DESKTOP III in 2021 and SOC-1 in 2024 found the opposite — surgery extended median OS by roughly 7-8 months. GOG 213 relied on the investigator's judgment. DESKTOP III and SOC-1 used validated systematic scores. Their complete gross resection rates were 67% in GOG 213 versus 75% and 77% in the two trials with systematic selection. Patients left with no gross residual lived longer than those who never had surgery at all. Patients left with any residual disease lived shorter than those who never had surgery. The individual patient-level meta-analysis by Gulia and colleagues confirmed it. The HR for gross residual versus no surgery was 1.72. The AGO DESKTOP I data is even more sobering. Residuals of just one to ten millimetres produced the same poor survival as residuals over ten millimetres. There is NO almost complete resection in the recurrent setting. Four scoring systems guide selection — MSKCC, AGO, Tian's iModel, and the Gemelli algorithm. They were derived almost entirely from patients who underwent primary debulking surgery. Their applicability to today's landscape — progression on PARP inhibitors or bevacizumab maintenance, BRCA-mutated relapses, prior NACT-IDS — remains uncertain. Neither ESGO–ESMO–ESP 2024 consensus nor NCCN addresses surgery after maintenance therapy or after initial interval debulking. Emerging data is beginning to fill those gaps. The MSKCC Team Ovary group has shown that prior treatment modality — primary debulking versus interval debulking — does not predict secondary cytoreduction outcomes when complete gross resection is achieved. A multicentre cohort published in 2026 found that BRCA-mutated patients gained nine months of progression-free survival and eighteen months of overall survival when surgery was added to platinum and PARP inhibitor therapy. CHIPOR, published in Lancet Oncology in November 2024, became the first level-one evidence supporting HIPEC at the time of secondary cytoreduction, with a hazard ratio for overall survival of 0.73. And in patients deemed unresectable upfront, six cycles of chemotherapy converted roughly 30% into surgical candidates, with meaningful survival gains in those who responded. The take-home is that secondary cytoreduction belongs to a carefully selected few. The decision rests on three pillars: a validated score to enforce objective selection, clinical judgement to refine it for the patient in front of us, and a goal of no gross residual disease. #GynaecologicOncology #OvarianCancer #SurgicalOncology #ASOOCON2026 #EvidenceBasedMedicine #CytoreductiveSurgery #WomensHealth #CancerCare #Odisha #Bhubaneswar

Israeli observability startup Coralogix has raised $115 million in a Series E round, becoming a unicorn with a valuation of over $1 billion. Coralogix offers a full-stack observability and security platform that helps companies understand and resolve complex issues in their data streams. The company has developed an AI agent, Olly, which uses a semantic layer over internal and external data to automate data observability and provide sophisticated insights through simple text prompts. The all-equity round was led by NewView Capital, with participation from the Canada Pension Plan Investment Board (CPP Investments | Investissements RPC) and NextEquity Partners, alongside existing investors. The funding will be used to expand its engineering base in India, with a planned $100M investment over five years, and to further develop its AI agent. In the months since, the pace of change has been remarkable. Platforms such as Snowflake, Databricks, Dremio, RisingWave, ClickHouse, Cloudera, Lakekeeper, Trino Software Foundation, and Amazon Web Services (AWS) have delivered significant new integrations, and Apache Iceberg itself has reached version 1.10.0, introducing capabilities that reshape how practitioners interact with the format. To reflect this momentum, we are undertaking a ground-up rewrite. Our priorities include: ✅ Iceberg 1.10.0 alignment and deprecation cleanup – removing outdated recipes that no longer reflect best practices while upgrading all demos, examples, and exercises to incorporate the changes introduced in Iceberg 1.10.0. For example, Snowflake’s support for catalog-linked databases to external catalogs makes earlier object integration recipes obsolete. ✅ Ecosystem validation – testing and refining recipes across Databricks, Snowflake, Dremio, and AWS to ensure seamless interoperability with Iceberg 1.10.0. We expect this rewrite to take two to three months. The H-1B system is finally back online. After nearly a month-long freeze, the U.S. Department of Labor has restored the FLAG portal and labor certification systems. For thousands of skilled professionals, most of whom are from India, this restart is more than a policy update. It is a lifeline. → Employers can now file new Labor Condition Applications (LCAs). → PERM certifications for Green Card sponsorship have resumed. → Delayed applications from the shutdown are finally being processed. During the shutdown, entire projects came to a halt. Tech companies couldn’t onboard engineers. Hospitals couldn’t hire critical staff. And thousands of visa holders faced expiry deadlines with no clarity. No, those systems are back up, and applications are moving again. However, this crisis reveals the fragility of the U.S. immigration pipeline. The Ayushman Bharat Digital Mission (ABDM) and PM-JAY are bold initiatives—digitizing records, integrating claims, and bringing millions into a digital health ecosystem. But here’s the hard question: Are we moving as fast on privacy as we are on digitization? A recent look at the PM-JAY dashboard reveals just how much sensitive information is out there: – Names – Hospitals – Discharge dates – Out-of-pocket payments All the details are available without a login. India’s Digital Personal Data Protection Act introduces fresh classifications of “sensitive” information that all stakeholders are still decoding. While the legal framework evolves, technology must proactively reinforce privacy. Health data isn’t just numbers. It’s a deeply personal story—one that can be triangulated, profiled, and (in the wrong hands) misused. This is where technology must do more. At Vitraya, we believe digitization and privacy should go hand in hand. Our use of Blockchain isn’t just for speed or automation.
